Extensive Application and Treatment Methods of Peripheral Vascular Interventional Therapy

1. Clinical application of peripheral vascular intervention therapy

At present, the clinical application of interventional therapy for peripheral vascular disease in China is becoming more and more extensive. Interventional therapy has the advantages of simplicity, safety, minimal trauma, fewer complications, accurate positioning, and repeatability. The scope of interventional treatment of peripheral vascular disease has developed from the original limb arteries to large arteries and veins, and even almost all blood vessels in the body.

The most successful interventions are the stenosis of the iliac, femoral, renal, and inferior vena cava. The scope of peripheral vascular interventional therapy is mainly vascular stenosis or occlusion, vascular dilatation and vascular malformations.

2. The way of peripheral vascular intervention therapy

At present, the main interventional treatment methods for peripheral vascular disease are balloon angioplasty and vascular stent placement.

The best indication for balloon angioplasty is limited, isolated short-segment stenosis of medium-sized or large vessels, followed by multiple, scattered short-segment stenoses and occlusions. Long segment stenosis or occlusion, small vessel disease, ulcerative stenosis, or calcified stenosis or occlusion are not candidates for PTA therapy.

Vascular stent refers to the purpose of placing an inner stent in the diseased segment on the basis of the expansion of the lumen balloon to support the narrowed and occluded blood vessels, reduce the elastic retraction and remodeling of the blood vessels, and maintain the smooth blood flow in the lumen. Some stents also have the effect of preventing restenosis.

In addition, interventional catheter thrombectomy and indwelling catheter contact thrombolysis can be used for arterial and venous thrombosis. Compared with surgery, it has less risk, fewer complications, strong patient tolerance, and better effect than medical drug treatment. In the treatment of aneurysm, a covered stent is used to isolate the tumor body for local intraluminal angioplasty, which can keep the vascular lumen unobstructed and greatly reduce the risk of aneurysm rupture and bleeding.

With the continuous development of interventional radiology, peripheral vascular intervention has undergone a process of continuous improvement and improvement from percutaneous transluminal angioplasty (PTA) to the development of dynamic angioplasty, laser angioplasty and intravascular stents. Good results; from the 1990s to the present, in order to effectively prevent restenosis after interventional therapy (restenosis rate of 30%), the research and application of gene therapy and intravascular radiation therapy have arisen, and certain results have been achieved, research in this area will continue in the 21st century.

It is foreseeable that with the continuous advancement of interventional equipment and new technologies, the continuous expansion of interventional scope, and the continuous improvement of interventional treatment methods, peripheral vascular intervention will play an increasingly important role in clinical treatment.
